How do i get the cape in the skull forest in the under world

i dont understand when ur in the desert cave thing and i can't find a place to go to find the little thing u've gotta collect

How do i find the swordsman helper. i get into the well unless he tempers with my sword.

in the items menu how/where do I find the item that is left of the magic cape?

i have 2 keys in the ice palace in the dark world but i can't find the doors i seem to be going in circles can someone tell me how to pass it to get to the boss????
