This FAQ is for Kingdom Hearts. It will provide information on
synthesizing items. The real purpose of this FAQ though, is to help
acquire the Ultima Weapon, Sora's most powerful Keyblade. Since you
need to mix all 24 items first to get the Ultima Weapon, I decided to
add the other Mix Ingredients. As I said there are 24 items in total
you need to mix before you can synthesize the Ultima Weapon. There are
5 categories or lists. Each is unlocked by mixing a certain number of
items.
List 1 – already there
List 2 – mix 4 items
List 3 – mix 10 items
List 4 – mix 15 items
List 5 (Ultima Weapon) – mix all 24 items
The Synthesis Shop isn't accessible until you get the Green Trinity
Skill.  Once you get this skill, go to Cid's Accessory Shop and use the
skill. Now you will be able to synthesis items when you talk to the
Moogle. You can unlock a few lists as you progress through the game
since you can acquire some ingredients. I advise you try to synthesize
everything after sealing Hollow Bastion. This is when you can actually
get all the ingredients you need. Hope this FAQ helps you. Last of all,

~1.4 Mushroom Strategies~

There are three different kinds of mushrooms. They are the White
Mushrooms, Rare Truffle, and Black Fungus. All of them drop the rare
Mystery Goo. And each also drops other things. They can only be
defeated a certain way and some may even retreat if you fail to use the
certain way. Here are some strategies.
White Mushroom
-=-=-=-=-=-=-=
They play a game of charades. They do a certain posture and you must
use a certain spell on them while they do it. If you fail, they will
retreat. If you take too long, they will retreat. Here are the correct
spells.
Fire – it shivers
Blizzard – it bends down and waves its hands
Thunder – a light hovers above there head
Gravity – they float off the ground
Cure – they fall down
Aero – they spin in place
Stop – they stop moving completely.
The last spell you cast on them that defeats them will nab you a gem
or shard of that element. For the shard, it's 100% and for the Gem, its
10%. You also have a 10% chance of getting Mystery Goo. If you kill
them using a spell three consecutive times, then you chances are
doubled for everything.

Black Fungus

These are just a pain to kill. They are the really dark Mushrooms.
They emit poison when you fight them. They can also petrify themselves,
which make them temporarily invincible, so back off until it wears off.
If you kill one, there is a high chance you may get a Mystery Goo and
Mystery Mold, especially if you kill them with a critical hit.

Rare Truffle

These are very tedious creatures. They will leap into the air. You
must keep them juggled by hitting them. Luck and timing is a very
important role. You should case Aero on your self too because the
shield bounces them off. If you hit them and they land on the ground,
they will retreat. If you juggle them 10 times, you will receive an
Elixir (100%) and Mystery Goo (20%). Hit them 50 times and you will get
Mystery Goo (40%), Megalixir (100%), and Shitake Rank. Hit them 100
times and you will get Mystery Goo (100%) and Matsutake Rank.
