My fave form is kingdom form, i got all these.

Unlockable:How
Masterget from da King b4 u go back wif tron
ValorGet when u change clothes
Finalmeet certain conditions (lvl up to certain lv or get via tournament in Olympus)
wisdomget i think after u do the 1st beauty and da beast lv
kingdomget when u have all summons/ forms at full lv.

u can also get final by getting any form up 1 lv and it will suddenly appear. (u'll turn into it while in form mode.)
