These rare equipment items can be obtained on HARD or CHAOS by playing the stage specified (non-Redux versions where applicable), with any of the characters specified in your party (you do not need to actually use the specified characters to achieve the necessary K.O.s). Once you achieve the required K.O.s, you will get a battle message stating that treasure has been found (you can check the Battle Log if needed), and a gold-colored item box will spawn near where you achieved the required K.O.s, so look for it and grab it before finishing the stage.
