When laying out it must have the format of this:

Name - what you would call your Keyblade
Appearance - what your Keyblade would look like
Ability - what ability your Keyblade has. The ability must come from Kingdom Hearts, Kingdom Hearts II or Kingdom Hearts Birth By Sleep has to be a Support abillity, and the number of abilities is limited to one per Keyblade.
Strength - how many points will be added to the Strength stat when equipped. The value must be between 0 and 9
Magic - how many points will be added to the Magic stat when equipped. Again, the value must be between 0 and 9
How to obtain it - how your Keyblade would be obtained in-game

1. Defeat Mysterious Figure first than Vanitas Sentiment if you dont to waste your time.
2. Keyblades in this x-Blade are Void Gear and No Name.

Name - Scrapped Artifact
Appearance - Themed after Wall-E
Ability - Does more damage with each successive hit in a combo.
Strength - 4
Magic - 6
How to obtain - Finish the "Axiom" world.

Best I could think of in 1 minute with what limited knowledge I have of how KH works.

Name: Organization's Fall
appearance:
Ability: Boosts all your stats to the maximum
Strength 9
Magic: 9
How to obtain: Secret Mirage Arena fight - only available to Ventus, become level 30 in mirage arena and defeat all arena missions and get Ultima keyblade - there is now a new selection called future fight. There are 14 rounds in this Arena Mission - Defeating replica's of the organization - until the 14th round where you fight all 13 members at once. Defeat them to unlock this keyblade.

Name: Masamune

Appearance: a shortened version of sephiroth's sword (it still has the largest reach in the game) with a second blade coming from near the tip towards the base to form the head. hand guards are similar to the one winged angel from CoM. Keychain is a six-pointed star with one black point (based on safer-sephiroth).

Ability: Hastega. All physical attacks are preformed at 2.5X speed.

Strength: 9
Magic: 8
How to obtain: defeat Sephiroth in the mirage arena. After the unlockable fight in radiant garden, (not real, just so no one flames me) fight sephiroth in the mirage arena. Beware, as Sephiroth has an attack that will cancel out all of your abilities.

Notes: a strong keyblade with incredible reach and a light weight.
(Sorry about the ability, I couldn't think of anything else that seemed to fit)

I am hoping that future games will have a keyblade based on the masamune.

Appearance: Make it look like yin n' yang but make it red and white not black and white.

Ability: Combo Boost

How to obtain: (custom mission) find the secret world of Gensokyo and battle with Reimu

Appearance: make it look like Flandre's Lavatein spell card on Touhou 6; Embodiment of Scarlet Devil
if you don't know Flandre's Lavatein spell card, search it on google.

Ability: Private Square; makes all of the enemies slow a little bit.

Strength: 8
Magic: 9

How to obtain: (custom mission)find the secret world of Gensokyo then go to the SDM, battle with Sakuya and
survive her last spell card: Deflation World.
Then battle with Flandre and beat her.
