 leave the leech monster alone with blood packs in a room, then keep making ur way to the water way. Once that leech god-zilla thing comes rig a nearby TNT or whatever barrel on fire and burn that S.O.B.

 you can't just kill the leech monster, find a lot of blood packs and drop them on the floor in a room and let him be.

 conserve your flask, and take out the dual colts and blast him the friking face a lot in quickdraw. P.S. learn his route when he's on the horse and get into a nice position to blast him.
