Aquire all conflict diamonds in the game available through payment or exploration

You'll need to find all 221 briefcases and needed to have done all payed missions and that give you a total of 1000 conflict diamonds. The paid missions are all the Main Missions and Cell Tower Mission.

Save a Buddy from captivity

You will save you first buddy from captivity right at the start of the game. (Mission 2) You mission is to go to a slaughter house and release the hostage, he's in the house at the back. Once this has been done you will get this achievement.

Work with a Buddy to subvert a mission offered by a faction

The first chance you get to get this mission is Main Mission 4 as long as you don't kill all your buddies before hand. This is the mission where you have to Assassinate a local chief constable for the APR. You can ether intercept his car and kill him or if you want the achievement your buddy will phone you offering you a better way of killing him, do your buddies way to get the points.

Find all the Safe Houses in Leboa-Sako - the Northern Territory

Safe Houses are marked on your map as a tent, some are guarded by a faction, and some are abandoned. It is smart to approach them with caution.

Find all the Safe Houses in Bowa-Seko - the Southern Territory

Safe Houses are marked with a tent on your map, like the other one. On your map if the icon has a padlock on it you must unlock the Safe House, usually by killing several enemies guarding it.

Turn over the first set of the Jackal's audio logs to the Journalist

You unlock this achievement the first time you turn over tapes to the reporter.

There are a total of 17 Jackal Tapes in the game: 1 that the Journalist gives to you, and 16 for you to find. There are 8 in the North and 8 in the South, with one in each square part of the map except for the center square in the North (where the city is), and the Southeastern square in the South (where the bar is).

Turn over all of the Jackal's audio logs to the Journalist

When you hand over all 17 tapes, you'll unlock Pulitzer Material for 20 points

There are a total of 17 Jackal Tapes in the game: 1 that the Journalist gives to you, and 16 for you to find. There are 8 in the North and 8 in the South, with one in each square part of the map except for the center square in the North (where the city is), and the Southeastern square in the South (where the bar is).

You basically only get two chances to turn tapes into the Journalist, once before you leave the North, then once in the South. Both times he'll appear sitting at a table in the Bar with the rest of your buddies.

The Journalist seems to appear almost at random in the North, but usually does near the end of the storyline in the area, or once you've accumulated enough tapes. Just to be sure, when you get a mission to assassinate someone at Goka Falls, don't take the mission yet and finish off collecting the Northern tapes, and hopefully the Journalist will have appeared.

When you go to the South, the Journalist should automatically appear once you've got all 8 tapes in the area. If he appears before you have all of the tapes, don't approach him as he may not appear again and you'll lose out on the achievement.

What most people don't notice is that if you miss some in the North, you can go back and get them. HOWEVER, when you pick them up, they will not play their audio, nor will they be added to your list in the pause menu. Because of this, you'll have to make sure on your own that you've collected all of them in the North before handing over the last batch to the Journalist.

Unlock new weapons by hitting a Convoy

After you are told that the tutorial is over you can visit your local weapon dealer. Go up to him and press Y for a side mission. After you blow up the truck or damage it heavily the mission is complete, and you will get this achievement.

Escape the Town

After the cut scene at the the start of the game you need to try and escape from the town. The quickest and easiest way to get this is to lose all your heath, you then get picked up and driven out of town.

Escape to Bowa-Seko - the Southern Territory

After you have picked the Church or your buddies in the final mission of the North Territory you will have to do one mission in the desert. When this is done you are told to meet Voorheer in Sefapane which is in the South Territory, you will then get this achievement.

Complete a Mission intercepted from a cellular tower signal

Find a cellular tower and find the box on the side, press Y to accept an assassination mission from an unknown caller, once the target is dead you will get the achievement.

Deliver stamped travel documents to the Underground in exchange for medicine

Once you run out of medicine, you can accept a mission from the priest to deliver some documents. You have to deliver the documents to some refugees in a veterinarians clinic, a good way to do this is to take the boat from the bar and go down the river at night. After you completed the mission the achievement unlocks.

Break the ceasefire and reignite the conflict

You get this after you do the first proper mission in the South Territory when you have to protect the barge. Successfully keep the boat intact until you reach the port to unlock this achievement.

Complete a Buddy side quest

Return to Mike after the tutorial and your 1st buddy will give you a side quest, finish the mission to unlock this achievement.

Get rescued by a Buddy

Once you have got your 2nd buddy from Mike's bar go and die and your buddy will come to rescue you.

Enter every 1 square kilometer map region of the world

You will probably get this in your process through the game just run around everywhere and you will eventually get this. You need to enter every square on the map there is 19 in total, 9 in the North, 9 in the South and 1 in the Desert.

Purchase the manuals and acquire the bandoliers for every weapon

You can buy the manual for each weapon once you have purchased that weapon, just highlight the weapon and press LB/RB to scroll through.

The Bandoliers appears in the upgrade list to purchase, just use RB to get to the other lists in the computer.

Switch sides in Leboa-Sako - the Northern Territory

In the Northern Territory just finish a mission for each of both factions, the achievement unlocks after you finish a mission for the second faction.

Switch sides in Bowa-Seko - the Southern Territory

In the Southern Territory just finish a mission for each of both factions, the achievement unlocks after you finish a mission for the second faction.

Entry Location:
Act 2: near Tae Moco Mine.

In Act 2 there is a small hut on the road just to the East of the mine with a Jeep beside it. On the the side of the hut with the Jeep break the boards over the window and climb in. In the middle of the hut is a Golden AK47. Enjoy.

Complete all the available Underground side-quests in both the North and the South

These are missions you have to do to get Medicine there are 3 Missions in the North and 4 in the South.

Purchase the manuals and acquire the bandolier for one weapon

Got to the computer in a gun shop, once you have bought a weapon you can buy manuals and a bandolier(ammo increase) for it, press RB to switch to the lists for these things.
